Alcohol-based hand sanitizer is the most commonly used hand hygiene product in healthcare — and also the most commonly misused. Understanding how sanitizer works, what it covers, and where it fails makes you a more effective infection preventionist.

How Alcohol Sanitizer Works

Alcohol-based hand sanitizers work by denaturing proteins — essentially destroying the structural integrity of microorganism cell walls and membranes. This is highly effective against most bacteria, many viruses (including influenza, COVID-19, HIV, and hepatitis B), and many fungi. The alcohol concentration matters significantly.

⚠ Minimum 60% alcohol required
The CDC recommends hand sanitizers with at least 60% alcohol. Products with less than 60% alcohol are significantly less effective — studies show they reduce microbial counts on hands but do not eliminate them adequately. Always check the label. Many household sanitizer products fall below this threshold, particularly those marketed as "gentle" or "moisturizing."

Correct Sanitizer Application Technique

Apply sufficient product

Use enough sanitizer to cover all hand surfaces — typically a dime-to-quarter sized amount in the palm. Too little product evaporates before completing the process.

Rub all surfaces

Rub palm to palm, between fingers, backs of hands, thumbs, and fingertips — the same areas addressed in handwashing. The rubbing action distributes the product and aids effectiveness.

Rub until dry

Continue rubbing until the hands are completely dry — approximately 20-30 seconds. Do not wipe hands on clothing or a towel before they are dry. If hands dry in under 15 seconds, you likely used too little product.

What Sanitizer Does NOT Cover

🚨 Sanitizer limitations — know these
  • C. difficile spores: Alcohol does not kill C. diff spores. If your client has C. diff, use soap and water every time.
  • Norovirus: Alcohol has limited effectiveness against norovirus. Soap and water is required.
  • Visibly soiled hands: Organic material (blood, stool, body fluids) inactivates alcohol and prevents it from reaching organisms underneath. Soap and water first.
  • Some non-enveloped viruses: Alcohol is less effective against certain non-enveloped viruses than against enveloped viruses. When in doubt, soap and water.
  • Bacterial spores in general: Most alcohol formulations do not kill spores — they kill vegetative (active) bacteria but not their spore forms.

Sanitizer Safety in the Home

  • Storage away from children: Alcohol-based sanitizers are toxic if ingested and attractive to children due to their gel consistency and often pleasant scent. In a client's home with children or confused clients, keep sanitizer in your bag, not on open surfaces.
  • Flammability: Alcohol sanitizer is flammable. Do not use near open flames. Allow hands to dry completely before any task involving heat.
  • Skin sensitivity: Some clients and caregivers have skin reactions to sanitizer components. If regular sanitizer causes skin irritation, try a fragrance-free formulation or contact your agency for alternatives.

When unsure: Soap and water is always appropriate. Sanitizer is sometimes appropriate. Default to soap and water when in doubt.
