The client's environment is an active participant in infection — not neutral background. Contaminated surfaces serve as reservoirs that silently transmit organisms every time someone touches them.
Section 01
How Long Pathogens Survive on Surfaces
⚠ Surfaces are active reservoirs
- MRSA: up to several weeks on dry surfaces
- C. difficile spores: months
- Influenza A: up to 24 hours on hard surfaces
- COVID-19: up to 72 hours on plastic and stainless steel
- Norovirus: up to 2 weeks on surfaces
Section 02
High-Touch Surfaces — Priority Areas
- Bed rails and adjustment controls
- Call buttons and remote controls
- Door handles and light switches
- Bedside table and items on it
- Telephone and communication devices
- Wheelchair handles and controls
- Bathroom fixtures — toilet handle, faucet handles, sink edge
- IV poles and pump surfaces if applicable
Section 03
Cleaning vs. Disinfecting
Cleaning
- Removes visible dirt and organic material
- Uses soap/detergent and water
- Mechanical action required
- First step before disinfection
Disinfecting
- Kills most organisms on surfaces
- EPA-registered disinfectant
- Requires dwell time (contact time)
- Not effective on visibly dirty surfaces
💡 Dwell time matters
A surface wiped with disinfectant and immediately dried is NOT disinfected — the product needs contact time to kill organisms. Read the product label (typically 30 seconds to 3 minutes). The surface should appear wet for the duration of dwell time.
Section 04
C. Difficile Requires Bleach
🚨 Standard disinfectants do NOT kill C. diff spores
Quaternary ammonium compounds (quats), most commercial disinfectant wipes — do NOT kill C. diff spores. For C. diff clients use:
- Bleach solution: 1:10 dilution (1 part bleach to 9 parts water, made fresh daily)
- Or an EPA-registered C. diff sporicidal product
- Bathroom used by C. diff client requires bleach cleaning after every use
Section 05
Cleaning Frequency
✓ When to clean what
- High-touch surfaces: At each caregiver visit — more frequently for contact-precaution clients
- Body fluid spills: Immediately — before it dries and before anyone else contacts it
- Bathroom: After each client use for contact-precaution clients; daily for others
- Equipment: After each use, before using with another client
- Wound care working surface: Before and after wound care
