Every safe transfer begins before you touch the client — it begins with how you position your own body. Your posture and stance are the foundation that determines whether a transfer is safe or dangerous for both of you.

The Athletic Stance

The starting position for any transfer or mobility task is what sports coaches call the athletic stance — and what body mechanics instructors call the power position. It is the most stable, strongest position the human body can assume:

  • Feet shoulder-width apart or slightly wider: Creates a stable base. For transfers, one foot may be slightly in front of the other (stagger stance) to allow forward-backward weight shift.
  • Knees slightly bent: Engages the quadriceps and glutes — the body's largest, most powerful muscles. Straight knees remove these muscles from the movement.
  • Hips back: The hips hinge backward to initiate lowering — this is the movement pattern that protects the lower back.
  • Spine neutral: Natural curves maintained — not rounded forward or hyperextended backward. A neutral spine distributes load across all the supporting structures rather than concentrating it on one area.
  • Shoulders back and down: Prevents the rounding that leads to upper back and shoulder injury.
  • Head up: Looking forward (not down at your feet) maintains cervical alignment and balance.

Where to Position Yourself

Close to the client

Your body should be as close to the client as possible throughout the transfer. Distance multiplies the load on your back. If you find yourself reaching — move closer before proceeding.

At the correct height

Working surface height should allow you to maintain neutral spine. Adjust bed height before transfers. If the surface is too low and can't be adjusted, widen your stance and hinge more deeply at the hips — never bend at the waist.

Facing the direction of movement

Position yourself so the transfer moves toward or alongside you — not across your body. If the transfer requires a pivot, pivot your feet during the movement rather than twisting your spine.

Weight Transfer — How Power Is Generated

💡 Use your legs, not your back
The power for any lifting or transfer movement should come from the legs — not the back. This is not a metaphor; it is a mechanical fact. The quadriceps and gluteal muscles are the largest, most powerful muscles in the body. The erector spinae muscles of the lower back are comparatively small and not designed for primary load bearing.

The movement pattern: shift your weight backward (loading the legs) → push through the heels → extend the hips and knees → the client rises. Your back is a stable pillar; your legs are the engine.

Footwear for Caregivers

  • Non-slip soles: Essential for transfers. A caregiver who slips during a transfer causes a fall for both themselves and the client.
  • Closed toe and heel: Open shoes reduce stability and cannot be used safely for transfers.
  • Low heel: Any heel height shifts the center of gravity forward, reducing stability in all directions.
  • Supportive sole: Standing for extended periods and repeated transfers require adequate arch support to prevent foot, knee, and hip fatigue that compromises form.

Preparing the Environment

  • Receiving surface positioned and ready (wheelchair brakes locked, footrests up)
