Understanding transmission, implementing appropriate precautions, and educating clients and families are essential home health competencies.

The MDRO Landscape in Home Care

Multidrug-resistant organisms are microorganisms that have developed resistance to multiple classes of antibiotics, making infections difficult or impossible to treat with standard therapies. The most common MDROs encountered in home care: MRSA (methicillin-resistant Staphylococcus aureus), VRE (vancomycin-resistant Enterococcus), CRE (carbapenem-resistant Enterobacteriaceae), and ESBL-producing organisms (extended-spectrum beta-lactamase). Home care clients often arrive from hospitals or long-term care facilities already colonized โ€” making home care the final point in the transmission chain where vigilant infection control can prevent household spread.

MRSA in Home Care

MRSA is the most commonly encountered MDRO in home care. It exists in two forms: healthcare-associated MRSA (HA-MRSA), typically causing wound, bloodstream, and pneumonia infections, and community-associated MRSA (CA-MRSA), typically causing skin and soft tissue infections. Both require contact precautions. Key transmission prevention: contact precautions (gown and gloves), meticulous hand hygiene (soap and water before leaving the client's home is advisable), dedicated equipment when possible, thorough equipment decontamination, and family education on not sharing personal items and proper hand hygiene.

VRE and CRE

VRE colonizes the GI tract and is transmitted via the fecal-oral route and contaminated surfaces. CRE includes several species (Klebsiella, E. coli, Enterobacter) that have acquired resistance to carbapenem antibiotics. Both require contact precautions. CRE is of particular concern because it is resistant to nearly all available antibiotics โ€” infections may be untreatable. Home care contact with CRE-positive clients requires scrupulous contact precautions, thorough environmental disinfection (EPA-registered product with adequate contact time), and careful documentation to alert subsequent care providers.

Client and Family Education

Educating clients and households about MDROs reduces transmission within the home and prevents spread to the community. Key teaching points: hand hygiene after contact with wounds or body fluids, not sharing towels, washcloths, razors, or other personal hygiene items, wound care technique and dressing disposal in sealed bags, laundry (wash MRSA-contaminated linens in hot water with detergent, dry on high heat), surface cleaning with household disinfectants, and signs of infection spread that require medical attention.
