Inspection protocol, neuropathy assessment, wound recognition, and client education
Diabetic foot complications are the leading cause of non-traumatic lower-extremity amputation in the United States. The tragedy is that most diabetic foot ulcers are preventable with consistent, systematic foot care. Home health clinicians are in a uniquely powerful position to teach, inspect, and intervene early โ often identifying problems the client cannot feel.
Three diabetes complications converge to make the foot uniquely vulnerable: peripheral neuropathy (loss of protective sensation), peripheral artery disease (reduced blood flow impairing healing), and impaired immune function (slower infection response). A client can develop a significant wound from a shoe rubbing, a corn being cut incorrectly, or a pebble in a shoe โ and feel nothing.
Teach clients to inspect their feet every day after bathing, before putting on socks and shoes. Assess all surfaces: top, bottom, heel, and between every toe. Use a mirror or ask a family member to help if visual access is limited. Look for: cuts, scrapes, blisters, redness, warmth, swelling, open areas, ingrown nails, and fungal infection (scaling, redness, peeling between toes). Any new finding requires reporting to the home health team.
At every clinical visit, perform a structured foot assessment: inspect all surfaces, assess skin integrity and moisture, check nail condition, palpate pedal pulses (dorsalis pedis and posterior tibial), assess capillary refill, and screen for neuropathy with a monofilament if available.
The 10-gram Semmes-Weinstein monofilament test screens for loss of protective sensation. Press the monofilament perpendicular to the skin at 10 plantar sites until it bends, then release. Ask the client to say when they feel it and where. Failure to detect the monofilament at multiple sites indicates loss of protective sensation โ flag for podiatry referral and escalate foot monitoring frequency.
Wash feet daily with mild soap and warm water (test water temperature with elbow โ neuropathy impairs temperature detection). Dry thoroughly, especially between toes. Apply fragrance-free moisturizer to dorsum and sole but not between toes (moisture between toes promotes fungal infection and maceration). Trim toenails straight across; file sharp edges. Never cut corns or calluses โ refer to podiatry. Wear well-fitted, closed-toe shoes with cushioned soles and no internal seams at all times, indoors and outdoors. Check the inside of shoes before putting them on. Never go barefoot.
The Wagner Wound Classification grades diabetic foot wounds from 0 (pre-ulcer) to 5 (gangrene). Any open wound on a diabetic foot โ regardless of size โ requires immediate clinical notification and complete wound documentation. Diabetic foot wounds can progress from superficial to infected to limb-threatening within 24โ48 hours. Early intervention dramatically improves outcomes and reduces amputation risk.
